The Relaxation Paradox

Here’s something that surprises most patients: muscle relaxation is not a good thing. In fact, for patients dealing with pain and injury, making muscles more relaxed, doesn’t address why they’re tight and slows recovery and can increase the risk of re-injury.

Massage, stretching, foam rolling, and percussion devices like the Theragun all share a core mechanism: they reduce muscle tone and create a temporary state of relaxation in soft tissue. That feels good. For many patients, it feels like treatment. But if the muscle being relaxed is tight because the nervous system is protecting an unstable joint or compensating for a weakened structure, removing that tension without restoring the underlying stability is like loosening the bolts on a wheel that’s already wobbling.

The Stretching Evidence: Worse Than You Think

Stretching is so embedded in athletic culture and physical therapy practice that questioning it feels almost heretical. The research, however, has been remarkably consistent over the past decade.

A 2025 international Delphi consensus of 20 stretching experts — one of the most authoritative panels ever assembled on the topic — concluded that stretch training does not prevent injury, does not improve posture, and does not serve as a meaningful recovery tool. Any improvements in range of motion from stretching occur through neurological tolerance adaptation — the nervous system becomes more comfortable with that range — not through structural changes to the tissue itself.

This matters enormously for clinical practice. If a patient has restricted hip flexion because the muscle is dysfunctional that range as a protective mechanism, stretching the hip flexors doesn’t address the root cause, it just temporarily pushes past the neurological boundary the brain set for a reason. The restriction returns within hours because the cause was never addressed.

Foam Rolling: Useful for Warmup, Not for Treatment

Research shows foam rolling produces short-term improvements in range of motion comparable to static stretching, and some evidence suggests it reduces delayed-onset muscle soreness (DOMS) after exercise. As a pre-workout mobility tool, it feels good. But, what is being sacrificed for that feel good sensation? Strength, and stability, what protects your joints, ligaments and connective tissue. Looking at strength changes, both rolling and stretching decrease muscle force output by over 25%. That is a huge problem when dealing with injury prevention, rehabilitation and sports performance.

The problem comes when foam rolling is used as a treatment for pain or a warm up tool. Like stretching, the ROM improvements it produces are neurological and short-lived — a 2024 meta-analysis found acute ROM increases from foam rolling and stretching were not superior to a general warm-up. And like massage, the ‘release’ it creates in a protective muscle pattern removes tension without addressing why that tension was there.

Massage and Percussion Devices: Treating the Victim, Not the Cause

Massage therapy produces effects: reduced muscle tone, decreased perceived soreness, and short-term pain relief. These are genuine physiological responses. They are also entirely symptom-focused.

The muscle that is ‘tight’ and responds to massage is almost always tight because it is being overloaded — asked to do work it wasn’t designed to do alone because another muscle is weak. Massaging that overloaded muscle reduces its tone temporarily. The inhibited muscle remains inhibited. The compensation pattern resumes. The tightness returns.

Percussion devices like the Theragun operate through the same mechanism as vibration massage — rapid percussive force reduces muscle spindle activity and creates temporary relaxation. This is a BIG problem since the muscle spindle is what regulate muscle function.

A 2024 systematic review and meta-analysis on self-massage (foam rolling, massage balls, percussion) found that while acute effects on pressure pain threshold were present, effects on muscle force production were negative and did not support the use of these tools as a primary intervention for improving neuromuscular performance.

Why Relaxed Muscles Can Increase Injury Risk

This is the point that most surprises patients: if a muscle is tight because it is protecting an unstable structure, and you relax that muscle without stabilizing the structure, you have just removed the body’s last line of defense.

This is particularly relevant before athletic activity. Static stretching before exercise has been shown to reduce muscle stiffness and power output — not because power is stored in tight muscles, but because the pre-stretch neurological state is part of what the muscle uses to generate force rapidly. Stretching before sprinting, jumping, or lifting can demonstrably impair performance and actually increases injury risk.

FAQs

What about after sports or hard workouts — isn’t foam rolling or massage helpful for recovery?

For post-exercise soreness and recovery, these tools increase the time to recover. Now the body has to overcome the workout and the forced relaxation. Remember the only way to get DOMS (soreness that persists beyond 72 hours after training) reduction, circulation improvement and recovery benefits is active and dynamic movement, not passive treatments. Also, if there is underlying dysfunction or injury present, using these tools without addressing the root cause delays proper assessment and treatment.
